When complete:
- Print a copy for yourself. This copy is good "insurance" for you; a copy may be required by your department's administration. If you haven't heard from MAES personnel within one week of submitting your form, call (612) 624-6971 to verify it was received.
- Send your completed form. You may fax your completed form to (612) 624-7724, mail it to 240 CofH on the St. Paul campus, or email it to dirmaes.umn.edu. Email by clicking the button at the bottom of the interactive page. If your email program does not open immediately after clicking the "submit form" button, this method will not work on your computer as it is set up. In that case, mail or fax a printed copy of the interactive page.
